What “Hormone Optimization” Means—And How It May Support Pain Care
At Midwest Pain & Wellness, we sometimes integrate Bioidentical Hormone Replacement Therapy— to address symptoms that can worsen pain conditions (lack of sleep, low energy, deconditioning) and to protect bone health. Care is individualized and evidence-based.
What Is “Hormone Optimization”?
“Hormone optimization” means achieving hormone levels that enhance your health and wellbeing. “Normal lab levels” may not be considered optimal for you.
How BHRT May Fit Into Chronic Pain Management
In our practice BHRT is not positioned as primary pain medication. But for the right patient, it can support a pain plan by improving sleep and menopause-related symptoms (which can enhance rehab participation), preserving bone density (lowering osteoporotic fracture risk), and improve muscle mass to support spine and joint stability. We combine BHRT with targeted PT, interventional procedures, and lifestyle improvement strategies.
Safety, Formulations & What We Use
We may use customized hormone formulations to help each individual achieve their goal. Therapy choice and route (transdermal vs oral, etc.) are individualized.
What BHRT may help with
May reduce aches and pains associated with aging. May also improve menopausal symptoms such as night sweats, hot flashes and mood swings; prevent bone loss and reduce fractures, increase lean muscle mass and improve bone density.
How We Help (3-Step Process)
1) Assess – Focused history, exam, and labs/imaging when needed.
2) Recommend – A comprehensive plan: lifestyle & rehab first; consider hormone therapy when appropriate; review risks/benefits and non-hormonal options.
3) Treat – Coordinate BHRT (when indicated) with our interventional pain management options and physical therapy; monitor outcomes and adjust. Why Midwest Pain & Wellness

BHRT FAQ's
What is BHRT—and are there FDA-approved “bioidentical” options?
“Bioidentical” means the hormone has the same molecular structure as the body’s (e.g., 17β-estradiol, micronized progesterone). Many bioidentical products are FDA-approved; major societies caution that compounded versions should not be used routinely when approved options exist.
Can BHRT reduce chronic pain?
BHRT is not a stand-alone pain treatment. However, in eligible patients it can improve menopause symptoms, sleep, and quality of life, which can make rehab and activity easier. We pair it with PT and interventional care for best results.
Will BHRT help prevent degenerative disease or bone loss? Will BHRT help prevent degenerative disease or bone loss?
In appropriate candidates, menopausal hormone therapy prevents bone loss and reduces fractures; that’s an FDA-recognized indication. It isn’t a blanket prevention tool for all “degenerative diseases.”
Does hormone therapy improve muscle mass?
In men with confirmed hypogonadism, testosterone therapy can increase lean mass and improve BMD (fracture reduction is unproven). We only consider it when labs and symptoms align.
Are pellets or compounded BHRT safer?
No. Compounded products are not FDA-approved and can vary in potency/quality; experts advise preferring FDA-approved formulations unless there’s a specific medical need to compound. We’ll discuss pros/cons and choose the safest option.
